    The 360-degree feedback form is intended to be utilized as a development tool in addition to an evaluation form and is based on feedback from supervisors, direct reports, coworkers, suppliers, customers or others who work with the employee/supervisor. This feedback is useful as it comes from beyond the typical chain of command — 360-degree feedback generally has more validity when the data comes from several sources versus only one or two people.
